Insurance Verification Responsibility

Actively present valid insurance cards and photo identification at each visit. Proactively inform our office of coverage changes before appointments preventing billing complications.

Deposit Requirements for Specialized Services

Strategically require $75 deposits for Hormone Replacement Therapy consultations securing appointment times. Definitively enforce non-refundable deposit policies for cancellations under 24 hours.

Medicaid Patient No-Show Consequences

Comprehensively implement 90-day scheduling suspension following Medicaid patient no-shows. Definitively discharge patients after three consecutive missed appointments without proper notice.
